BARRIERS TO PMTCT PROGRAMME IMPLEMENTATAION IN FREE STATE, SOUTH AFRICA INTRODUCTION: paediatric transmission of HIV infection occurs during pregnancy, labour and delivery or through breastfeeding. This is the most common mode of transmission to children in the world (1)Without specific interventons the risk of mother to child tranfer(MTC) is somewhat 15-30%, if the child is not breastfed (2) . World health organization (WHO) has guidelines to keep back MTC and the latest one of 2010 has wider options for feeding infants to prevent MTCT (3). Some of the unquestionable countries have successfully implemented this programme by nearly eliminating paediatrc HIV transmission (4) and the scenario is quite different in places like South Africa. Recently South Africa launched their national st ramblegical plan where they state to abridge MTC to<2% by 6 weeks after birth and < 5% by 18 months (NSP, 2012). Although they have an increased coverage of PMTCT reducing MTCT to < 4% (4)in some areas but there are some province like shift state where the there are barriers to its succession. Now Free conjure was found to have the highest rate of vertical transmission in the country at 6% with an exposure rate of 31.1% (4). The estimated MTCT rate here is 12.3% (5).
They also have the highest number of under 5 mortality rate in the country predominantly payable to AIDS(6). Without any measure 30% of the infants will be overt to HIV. Various studies have looked into ways of successful implementation of the PMTCT programmes and given up possible reasons behind failure of successful implementaion of it (2,9,10) . This study aims to check the contextual factors that hinder the access and adoption of PMTCT programme in Free State.
